Overview


Principle 1
We have clear accountability for personal information 
Baycrest is responsible for the personal information it holds, and adheres to the Baycrest  Privacy Code. The Baycrest Privacy Office oversees compliance with the following privacy principles.

Principle 2
We are clear about purposes for the collection of personal information
When Baycrest collects your personal information, we will identify (and if necessary, explain) the purposes for the collection at or before the time of collection. These purposes include: client care, administration and management of the health care system, administration and management of Baycrest programs and services, research, teaching, statistics, fundraising, and as permitted or required by law.

Principle 3
We ask for consent for the collection, use, and disclosure of personal information
Your knowledge and consent (or that of your legally authorized representative) are required for the collection, use or disclosure of your personal information, except where the law permits or requires otherwise. 

Principle 4
We limit the collection of personal information 
Baycrest will limit the collection of personal information to that which is necessary for the purposes identified. Information will be collected by fair and lawful means. 

Principle 5
We use and disclose personal information only for purposes you consented to
Baycrest will not use or disclose your personal information for purposes other than those for which it was collected, unless you consent, or as permitted or required by law. Baycrest will keep the information only as long as is necessary to fulfill those purposes. 

Principle 6
We maintain complete and accurate information 
To the extent reasonably possible, Baycrest will take steps to ensure that your personal information is as accurate, complete and up-to-date as is necessary for the purposes for which Baycrest uses it. 

Principle 7
We safeguard all personal information 
Baycrest will protect your personal information with security safeguards appropriate to the sensitivity of the information. 

Principle 8
Our privacy policies and practices will be made clear 
Upon request, Baycrest will provide you with specific information about our policies and practices relating to the management of personal information, including what information is made available to related organizations. 

Principle 9
You have a right to access your personal information, and request that it be corrected
You may ask for access to your record of personal information held by Baycrest, and will be given access within timeframes set by law and at a reasonable cost. You may also challenge the accuracy and completeness of the information and request that it be corrected. Baycrest may deny or restrict access and correction in limited circumstances, which you can ask to have explained to you.
