1. You will meet the Nurse Clinician who will:
- Review your health history and stroke risk factors
- Take your blood pressure
- Review what happened to you when you had your stroke
- Review with you the questionnaire you sent in
- Initiate the Care Plan with you and your family or caregiver
2. You will meet the Neurologist who will:
- Review your history and needs in consultation with the Nurse Clinician
- Review your stroke imaging (CT Scan and/or MRI results)
- Evaluate your thinking skills
- Contribute to the Care Plan
3. You may or may not:
- Be referred to other team members for further assessment
- Be asked to undergo further investigative tests.
