Referrals from the client's physician are required.
Referral Criteria
There is no age limit for referral of clients except they must meet the appropriate criteria which includes:
- Clients who have a diagnosis of a single stroke (embolic, lacunar (small vessel), or hemorrhagic) resulting in cognitive or higher-level motor impairment including memory, executive language, perceptual and attention/executive problems
- Clients who have suffered multiple strokes will not be excluded, but must have some expectation of benefit from the program
- Clients with primarily multiple infarct (vascular) dementia will likely be seen initially in the Memory Clinic.
Excluded diagnoses (clients not eligible for this clinic)
- clients cannot be bedridden
- clients who have been diagnosed with subarachnoid hemorrhages, as well as end-stage debilitation in other areas that would limit participation in rehabilitation, such as cardiac, diabetes, arthritis, etc.
Services provided
- functional assessment
- diagnosis
- interdisciplinary care (including Nurse Clinician, Neurologist, Occupational Therapist, Physiotherapist, Neuropsychologist, Social Worker, Speech Language Pathologist)
- counseling (MD to patient, not psychotherapy)
- consultation to the referring MD regarding pharmacotherapy and restorative therapy
What clients should bring to their appointment
- OHIP card
- all medications in their original bottles
- assistive devices (e.g. hearing aids, glasses, canes, walkers, etc)
