Baycrest Hospital – 10 in-patient units
3East
3E is a 27 bed acute medicine unit designed to meet the acute care needs of our current clients (Apotex, Hospital and Terraces). The unit has a 7-bed cardiac monitoring telemetry area with continuous BP and O2 saturation monitoring. The remaining beds are step-down and transitional care in nature.
Patients who utilize 3E (from our other areas) have a variety of medical needs that demand more complex nursing care. Typical skills that staff require on this unit include: the set up of cardiac monitoring, 12-lead ECG reading and monitoring, management of enteral feeds, administration of oxygen, management of intravenous infusions, multiple IV medications, management of catheters, and management of complex dressings and wound care. The patients are generally acutely ill frail elderly. They require not only specially skilled nurses, but also those who are caring and compassionate, with a focus on personhood as well as disease management.
3West
3W is a 34 bed unit, consisting of 24 designated geriatric rehabilitation beds with specific admission criteria. Patients are all over the age of 55 and have had orthopedic surgery, commonly hip and knee replacements, and/or are patients who have had general surgery, or have been on a medical unit with cardiac, respiratory or endocrine disorders resulting in becoming deconditioned due to length of stay and prolonged bed rest. The patient acuity can at times, be very high.
The focus for rehabilitation is to encourage patients to be as independent as possible, with the expectation that they participate in a program designed to meet their independence goals. The multidisciplinary team works in partnership with the patients to set these goals.
Within 3 West is an 8-bed Geriatric Assessment and Treatment Unit (GATU). The focus here is on assessment, treatment and evaluation of patients with extremely complex medical, functional and emotional problems. The opportunities for holistic, family centred, goal driven care abound on this unit.
4East- Inpatient Psychiatry
4East is a 20 bed in-patient unit caring for geriatric patients who have mental health challenges. The patients are predominantly female, mostly in their 70’s and 80’s, many of them Holocaust survivors. Many have suffered from depression for many years and in addition, have had to cope with medical, cognitive and social losses which are a further challenge. The unit provides a wide variety of treatment modalities, including electroconvulsive therapy (ECT).
4West- Behavioural Neurology
4West is a 20 bed in-patient unit specializing in the assessment and treatment of patients with degenerative neurological conditions with behavioural manifestations. The patients are of varying ages, are predominantly male, and are often very disturbed and agitated. Many of the patients are referred from other Long Term Care facilities as a result of behavioural disturbances.
The multidisciplinary team consists of nursing, neurology, psychiatry, social work, nutrition, speech and language therapy, therapeutic recreation, psychology and physiotherapy. Staff of all disciplines find this a stimulating learning environment.
Psychiatry Day Hospital
The day hospital offers a 12-week program for adults over 65 with mental health challenges. These adults are living in the community and the program goal is to assist this level of independent functioning. The program is multidisciplinary and works on a case management model. Here, staff have exposure to and an opportunity to practice group psychotherapy, cognitive behaviour therapy, and a variety of other group programs.
Complex Continuing Care Cluster - 5East, 5West, 6East, 7East and 7West ( 5 units)
These are all 34-bed units providing long stay care for older persons with complex medical conditions that cannot be managed either in the community or in long term care facilities. The cluster uses a primary care team model to support positive patient outcomes. This primary care team model focuses on the concepts of patient empowerment, enablement and choice – supporting personhood and combating against ageist actions or decision making. Staff have the opportunity to learn about these concepts and practice in a client centred environment. Issues of consent and capacity, helping families dealing with grief and loss, and gerontological nursing practice are also key practice issues on these units.
6 West - Palliative Care
The Shirley and Philip Granovsky Palliative Care Unit
Our beautifully renovated in-patient Palliative Care Unit at Baycrest provides 24-hour pain and symptom control combined with emotional and spiritual support for patients in need of palliative care. This 31 bed unit has beds reserved for both short-term palliative applicants (prognosis of 90 day or less) and long-stay palliative applicants (prognosis of up to two years).
The unit also offers Respite stay for up to two weeks to enable caregivers, within the community, to have relief from their care giving responsibilities. Respite patients receive pain and symptom control in a supportive environment. Please note, there may be some limitations or exclusions to some Baycrest programs and services.
