Ambulatory Care – 3 Areas
Clinics
This area includes approximately 25 specialist consultation clinics, including Endoscopy and Foot and Wound Clinics. Clinics occur at varying frequencies, from 1-6 clinics/day to 1-2/month. This area has three nursing led clinics – allowing staff to experience the full scope of RN practice.
Community Outreach Team (COT)
Clients who are referred to this team are usually experiencing a complex web of medical, functional and social problems that interfere with their well being and ability to function (for example: falls, difficulty bathing, swallowing problems and/or caregiver stress). The COT nurse visits clients in their homes to complete comprehensive assessments (physical, cognitive, social, and, environmental) and then refers the client to the most appropriate team member(s) for follow-up and short term treatment. These additional assessments and interventions happen in the clients’ homes.
Day Treatment Centre
The Day treatment centre is a Geriatric Day Hospital that provides rehabilitation and education in a programmatic format, to frail seniors living in the community, who are having difficulty functioning in their homes. The goal of treatment in the Day Treatment centre is to assist clients to attain and maintain optimal physical and psychosocial functioning. At discharge clients are usually linked to a community program. The services of this program are offered to older persons who have had a stroke, live with Parkinson’s disease and/or seniors with complex medical conditions, and who require the intervention of two or more health disciplines.
