Consultants to the Care Team
Social workers provide counselling services during the initial admission period and also during periods of change, transition and uncertainty. They consult with the Care Team on complex resident/family situations and may work to mediate and resolve difficult issues. They also help residents and families make decisions and interpret information.
During difficult periods, residents and families can request a referral for social work services. Please discuss such needs with your nurse or unit director.
The clinical nurse specialist is an advanced practice registered nurse who supports quality of care through staff education, consultation and research.
The nurse practioner is a registered nurse with advanced skills who works closely with the physician to assess and treat your medical needs.
The occupational therapist (OT) helps residents maintain their highest possible level of functioning when it comes to activities of daily living. This person provides expert assessments, prescribes seating and mobili¬ty devices and various assistive devices for daily activities such as bathing, eating and dressing. The OT may also recommend changes to the environment and provide specialized services such as prescribing hand splints and orthotics (shoe inserts). Our occupational therapy services are provided by a private outside company.
The physiotherapist (PT) helps residents maintain or improve their highest possible level of physical mobility and functioning by providing expert assessments, consultation and/or treatment. You may be assessed soon after admission to establish baseline mobility levels and to determine whether follow up is necessary. Our physiotherapy services are provided by an outside company.
The speech language pathologist (SLP) assesses residents who have difficulties with communication and/or swallowing. Our own speech language pathologists help with swallowing problems, while an outside Community Care Access Centre (CCAC) provides services for residents with aphasia (difficulty speaking and/or understanding words) and other communication disorders.
The audiologist assesses hearing problems and prescribes hearing aids or other assistive devices.
The psychologist and psychiatrist can, as appropriate, assess cognitive func¬tioning and then recommend possible treatment approaches.
The pharmacist ensures residents receive the correct medication(s) in an fitting manner and takes part in ongoing reviews of med¬ication(s) with the resident’s physician and members of the nursing staff.
The diagnostic imaging technologist provides imaging services based on orders and clinical information received from the physician. This may include general x-ray exams, bone mineral densitometry testing and ultrasound exams.
Our on-site Rabbi/Chaplain and other chaplains are available to provide religious and spiritual support to residents and their families. Community clergy are welcome to visit.
