Sometimes we need to get in touch with your family. That’s why we should always have up-to-date contact information. Please let your Unit Clerk or Registered Nurse know when relatives’ phone numbers (at home or work) change. It is also helpful for us to know what times of day they are normally available by phone. If the name of your contact person  changes – either for a short period of time or permanently – the Care Team should also be informed right away.
