Self-Management Programs for Patients and Caregivers
Self-management programs encourage people with chronic illness to improve their own health. Health professionals help clients and care partners develop the skills needed to manage their condition on a daily basis through developing problem solving skills, setting meaningful goals, and identifying resources and supports.

On-line Resources
Baycrest has a number of on-line resources including a support group, electronic newsletters and educational websites.

Support Groups for Community Members and Caregivers
Baycrest offers a number of support groups to community members and caregivers that address bereavement, dementia, experiences of Holocaust survivors as well as general support. For more information about caregiver support, call Seniors Counselling and Referral at 416-785-2500, ext. 2223.
