Admissions and Discharge Planning


Admissions

Moving into a geriatric facility can be a stressful time for an individual as well as his/her family members. Whether a person is entering the Baycrest Hospital, the Apotex Jewish Home for the Aged or the Baycrest Terraces, a social worker will help you with the transition.

Whether your admission has been planned or unexpected, there are routine procedures that you can anticipate. There are forms to fill out, questions to answer, staff to meet and new routines to get used to. Your social worker will help with this process and keep your family informed. The more you are able to share your concerns and needs with your social worker, the more he/she will be able to support you.

Your social worker is your link to your Care Team who will ensure that you get the care you need throughout your stay at Baycrest.

Discharge

If you come to the Baycrest Hospital to a short stay unit, your social worker will assist with discharge planning. Discharge planning is a team effort that is started at the time of admission.

The most important people involved in planning for your discharge are yourself, members of your family or close friends and all members of the Care Team. Other key participants are representatives from community agencies who will be arranging or providing services to you in the community. Your input will help us organize a plan that meets your needs and desires and that is reasonable for you and other members of your family.

Your social worker will help you find the appropriate resources in the community or another facility, based on your needs. Ask the social worker if there is any part of the discharge plan that you do not understand.
