Baycrest offers a variety of programs for seniors living in the community.
Seniors Support Program
This program offers support through weekly friendly telephone calls to older adults living in the community. The Seniors Support Program trains volunteers who enjoy using the telephone to reach out to older adults. The social worker coordinates the program, interviews new clients, and provides counselling and referral services as needed.Contact:

Baycrest Community Day Centre for Seniors
Baycrest Community Day Centre for Seniors is a unique day program for older adults who are physically frail or cognitively impaired and living in the community. Social workers at the Centre provide individual assessments, counselling and case management services for all members as well as supportive counselling for their family members.Whether older adults are members of the Parkland, Oceanside or Samuel Lunenfeld Mountainview Clubs, their social workers arrange for guest speakers, special interest group programs and daily "Morning Meetings" where new friendships can flourish and old friends are sustained. Social workers also provide counselling, family caregiver support groups, education and resource material and assistance with long term planning.

The Psychiatric Day Hospital for Depression
The social worker in the Psychiatry Day Hospital works with a multidisciplinary team to treat older adults suffering from depression who are living in the community. Social work services include individual and group counselling for the participant as well as supportive counselling for family members. Prior to discharge, the social worker will work with both the patient and the family to ensure that community supports and resources are in place.The Team social worker is:

Out-patient and Community Psychiatry Services
Mental health social workers are active members of the Geriatric Psychiatry Community ServiceSocial workers will see people either in their offices at Baycrest, in the West Wing, or in their own homes, and in both cases are part of a Team of professionals. The social worker offers individual and group counselling, appropriate referrals where necessary and will help individuals and their families coordinate services. These clients usually suffer from mental health problems such as depression and memory loss, personality changes, confusion, sleep disturbances and anxiety.

Community Assessment and Treatment Team (Day Treatment Centre)
The social worker in the Saul and Rae Saltsman Day Treatment Centre is part of a multidisciplinary team that assists older adults living in the community to obtain and maintain optimum physical and psychosocial functioning. Social work services include admission planning, individual counselling, support for family and caregivers and prior to discharge ensuring that appropriate community supports and programs are in place.The Team social worker is:

The Brain Health Centre Clinics
The Brain Health Centre Clinics social workers provide psychosocial assessments, counselling, education, and support to persons and their families living with memory, mood problems or stroke. This can include resource identification and referrals to community services, advocacy, as well as assistance with problem-solving and long-term care planning.- The Brain Health Clinics' central number is 416-785-4359

Community Outreach Team, COT
The COT Social worker will visit people in their own home or in the Clinic here at Baycrest. Services provided by the Social Worker include: talking to clients about their illness, assistance with long-term and helping caregivers deal with their stress. He/she will help both clients and family members connect with appropriate resources. This may include help at home, caregiver support groups, referrals to day programmes etc.The COT social worker is:
