Social workers play a key role in the following in-patient Programs:
In-Patient Rehabilitation:
This is a short-stay unit in the Hospital, where the social worker will help patients and families plan a safe return to the community.
While in the Hospital the social worker will provide assessment and counselling services. The social worker also serves as a liaison between the patient, the family and the rehabilitation team. Click here for more information about the Rehabilitation Unit.
Geriatric Assessment and Treatment Unit, (GATU):
This is a short-stay unit in the Hospital, where the social worker will help patients and families understand the assessment process, act as a liaison with the Assessment Team and facilitate the acquisition of community resources and services to ensure a safe transition home. In-Patient Psychiatry Program:
The social worker on the In-patient Psychiatry Program works with the Team to ensure that the patient and family members understand all aspects of treatment and that patient and family concerns and questions are addressed. He/she also provides individual and family counselling where required.

In-Patient Palliative Care Unit
The social worker on the Palliative Care Unit provides patients and families with emotional and practical support throughout a potentially difficult period. He/she will help both patients and their loved ones talk about their concerns and fears. The social worker, along with the entire Palliative Team, will provide an integrated, holistic approach to care, encouraging a support network among clients, family, staff and volunteers. The Behavioural Neurology In-Patient Unit
Social work on this Unit is part of a multi-disciplinary Team. The social worker provides individual and family counselling as well as providing family members with educational and resource material about the specific illness affecting their loved one. Prior to discharge, she will also help the family with long term plans, consider their options and help arrange for the necessary resources and services. Complex Continuing Care:
The social workers on the Complex Continuing Care Floors at the Baycrest Hospital will ensure that patients and their families receive the individual, family and group counselling as it is needed. The social worker works with patients and families to ensure that living in a hospital setting will be as comfortable, welcoming and enjoyable as possible. Social workers, as part of the Care Team, will work to enhance individual patients' strengths and abilities and support them in coping with their illnesses. Living in a hospital setting or having a loved one do so can provide challenges and the Complex Continuing Care social workers will support the family as they face these challenges. In-Patient Acute Care Unit (3E)
This unit is an inpatient short-stay unit for residents of the Baycrest campus including Baycrest's Terraces, Apotex and Complex Continuing Care Unit. These residents may be transferred to '3E' for treatment when their medical condition becomes unstable. The social worker on 3E works with the patient and family and provides psychosocial assessments, individual and family counselling and referrals where appropriate. The social worker is a part of a collaborative interdisciplinary Treatment Team, and will also work with the team from the patient's home unit to ensure appropriate delivery of services upon discharge.
