Family Satisfaction Survey

At Baycrest, we strive to provide the best possible care and service to our patients and residents. In order to do so, we need to know what we are doing well and where we may need to improve.

We have developed a survey for family members and representatives patients and residents at Baycrest to better understand and evaluate your experiences and the experiences of the patient/resident.  We do this because your opinion matters to us, and your feedback will help us to be even better at what we do.

The survey should take approximately 10 minutes to complete.

Your participation in the survey is voluntary and private. Your feedback is important to us, so we ask that you express your opinions frankly and assure you that your responses are strictly confidential. To protect your privacy, the data is being collected by the Department of Quality and no personal information will be provided to Baycrest. Feel free to involve other family members/friends when completing the survey or send the link to other family members as well.
