Accessing your health record

If you were a patient of Baycrest and would like to receive a copy of your health record, you can download and complete the Request Access to Personal Health Record form and either mail, fax or hand deliver it to our Health Records office. Your request will be processed within 30 days.

Confidentiality of patient information is very important to Baycrest. Your personal information will only be released to you, your substitute decision maker, legal representative or as authorized by law.

If you are a representative requesting a health record on behalf of a patient, you must also send a signed consent form.

Fees

If you would like a copy of the records for personal use, there is a $30 production fee as well as the following additional fees:

Photocopies: $0.25 per page after the first 20 pages
Microfilm and microfiche: $0.50 per page

There is no charge to send copies of a Health Record to another physician or health-care provider.
