Integrated Community Care Team
The Integrated Community Care Team is a geriatric outreach team that provides care for medically complex older adults in their own homes.
We visit people who are over 65 years old who are homebound with multiple serious medical conditions.
Our outreach team includes the following health-care professionals:
- Physicians including geriatricians and care of the elderly physicians, who focus on care for older adults
- Nurse practitioners
- Registered nurses
- Occupational therapists
- Physiotherapists
- Social workers
- Ontario Health at Home coordinator

Members of our team will visit you in your home to perform a comprehensive assessment that we share with your family doctor.
We will talk to you about your medical history and how you are feeling and do a physical exam. We will then work with you to decide on a treatment plan that is best for you.
We try to connect you with resources in your own neighbourhood to help improve your quality of life and offer support to your caregivers.
If you need more support, we also offer two options for on-going care:
- Shared care which means we will continue to visit you and work together with your family doctor
- Primary care which means we will continue to visit you and provide your primary care if you do not have a family doctor
- OHIP card or other health insurance information
- All medications and supplements in their original bottles
- A list of questions you want to ask
- Any aids you use, such as a walker, hearing aids or glasses
- A friend or family member for support, if you choose
You will need a referral from a health-care provider to receive this service. You may want to ask for a referral if you are experiencing any of the following:
- An increased number of visits to your family doctor
- Multiple hospital admissions
- Multiple visits to the emergency department
- Recent changes in your physical, mental or functional abilities
- Recent increases in falls, worsening mobility, incontinence or dementia

What is the integrated community care team (ICCT)?
The ICCT is a team of different health-care professionals who work together to care for medically complex older adults who live in the wider community. Our team will visit you wherever you are staying and help you find the right medical care and community support.
How often will the ICCT visit me?
If you are referred to the ICCT, the number of visits you receive will depend on your specific needs. Some people may receive more frequent visits at first and then only need occasional support moving forward. The ICCT also offers virtual visits.
Is there a cost for this service?
No, there is no cost. The ICCT is covered by the Ontario Health Insurance Plan (OHIP).
