Family Advisory Council 

The Family Advisory Council (FAC) is a group of family members and caregivers who meet monthly to support compassionate, high-quality care for residents at the Apotex Centre, Jewish Home for the Aged.


The Council:

  • Provides feedback on resident care and experience
  • Brings forward issues and perspectives from families and caregivers
  • Makes recommendations to Baycrest leadership, Ministry officials and elected representatives

 

FAC members may also participate in Baycrest committees and working groups, helping to ensure the family voice is represented across the organization.

To join the Council, members must have a relative or significant individual who is a current resident of Baycrest and be able to commit to a minimum one-year term.


Mission statement

The mission of the Apotex Family Advisory Council is to support and advocate for the delivery of comprehensive and compassionate care for residents of the Apotex Centre, Jewish Home for the Aged, in keeping with Jewish values and traditions as well as the scope outlined in the Fixing Long Term Care Act.

Learn more about the Family Advisory Council

The FAC:

  • Operates in an advisory capacity, recommending on matters that impact Apotex residents, their families and caregivers
  • Works in partnership with residents, families, residents and Baycrest staff
  • Considers issues and concerns that may affect multiple residents, rather than only individuals

The Council is guided by language defined in its FAC Terms of Reference and the Fixing Long Term Care Act.

The council represents Apotex families by:

  • Acting as a bridge between families and Baycrest teams to share feedback and perspectives.
  • Encouraging open communication by bringing forward family questions, ideas and insights.
  • Supporting initiatives that enhance the resident and family experience.
  • Providing feedback on programs, services and resources to support ongoing improvement.
  • Supporting education and orientation opportunities for families and caregivers. (Family Care Connection program)
  • Participating in activities that strengthen connections between residents, families and staff.
  • Advocating for resident and family perspectives by identifying common themes and opportunities.

Baycrest staff members provide the FAC with organizational and administrative support and are often present at monthly meetings, highlighting a strong and collaborative rapport with each other.

Families, residents and staff from each Apotex floor may nominate individuals to join the Family Advisory Council Family members interested in becoming involved may also contact the FAC chairs directly.

A maximum of six family representatives from each floor of the Apotex may serve as council members.

Term of Membership

Members commit to a minimum one-year term and may remain active for up to two years following the loss of their loved one.

The FAC Executive Council is comprised of a chair and/or co-chairs who are active members appointed annually by the council.

Members are expected to support the mission of the council, as well as Baycrest’s Values, Vision and Heritage Statement.

Regular council meetings are held multiple times per year with special meetings held as needed.

Family members are welcome to attend an initial meeting to learn more about the Family Advisory Council before deciding to become a member.
