Day Treatment Centre
The Saul and Rae Saltzman Day Treatment Centre is an outpatient rehab hospital program that offers interprofessional medical services for older adults with complex medical conditions.
The Saul and Rae Saltzman Day Treatment Centre (DTC) provides a rehabilitation program for older adults living in the community with complex medical conditions.
Clients in the program receive care from a team of clinicians, which may include a Geriatrician, Registered Nurse, Social Worker, Speech Language Pathologist, Physiotherapist and Occupational Therapist.
The program supports individuals who require care from at least four members of this interdisciplinary team and uses self-management principles to help clients manage their health.
A referral from a physician or Nurse Practitioner is required to attend the program.
Our compassionate interprofessional team works together to assist clients acheive their maximum functional potential and remain independaent for as long as possible.

Our program is designed to last up to 12 weeks. Sessions are held twice a week and are three hours long. You must commit to the program length before enrollment.
At referral, an intake appointment with our clinicians will take place. Once you begin the program, our team will work together to create a custom treatment plan that will help address your concerns and meet your goals. Clients who meet their goals early will finish the program sooner than 12 weeks.
We encourage you to be an active participant in the program. Clients who are self-motivated, willing to learn and follow our recommendations benefit the most.
The day treatment centre program includes the following:
- Setting rehabilitation goals
- Group sessions for exercise and education
- Clinical assessments and treatments
- Discharge planning
Please note that our programs and medical assessments are offered in English, translation services may be available if needed. For individuals needing assistance wtih personal care, translation or cognitivite support, they should bring a support person with them while enrolled in the program.
- OHIP card or other health insurance information
- A support person, if needed
- A list of questions you want to ask
- All the medication and supplements you take (first visit only)
- Any medication you might need to take while at the clinic
- Comfortable clothing for exercising
- Appropriate footwear for weather and exercise
You will need a referral from a physician or nurse practitioner to attend our program. The Day Treatment Centre is covered by the provincial health plan (OHIP).
To be eligible, clients must meet the following criteria:
- 65 years or older
- Benefit from four or more of our disciplines (i.e., geriatrician, nursing, OT, PT, SLP and social work)
- Under the care of a family doctor or nurse practitioner
- Able and willing to participate in a group setting
- Able to toilet themselves independently or with assistance from a family member or caregiver who is present at the program.
- Able to follow recommendations or instructions delivered in English, or have someone who can translate present during the program.
- Have ongoing and reliable transportation to and from the program
- Have a support person who can attend with them to help with daily tasks, if needed
You can not attend our program if any of the following apply:
- Under the age of 65
- Do not need four or more disciplines (i.e., medicine, nursing, OT, PT, SLP and social work)
- Require help with toileting and cannot be accompanied by a support person
- Have moderate cognitive impairment and cannot be accompanied by a support person
- Have disruptive behaviours and cannot participate safely
- Attended another geriatric day hospital within the past year and have no new goals
- Have special care needs that are better served by a different specialized program such as stroke or mental illness

Who can attend the Day Treatment Centre?
The Day Treatment Centre is for older adults who live in the community and are able to come to a outpatient-based program twice a week. Clients are experiencing medical and functional challenges and want to participate in therapy, exercise, education and self management to improve their condition. Please review the the inclusion and exclusion criteria for more information.
Is there a cost to attend the Day Treatment Centre?
The Day Treatment Centre is covered by OHIP. Clients need their own transportation to and from the program and bring a personal support person, if needed.
How often can I attend?
The Day Treatment Centre offers programming twice a week for up to 12 weeks. If you reach your goals early, you may finish the program before 12 weeks. Each session lasts for three hours.
