Interprofessional Primary Care Team

The Interprofessional Primary Care Team (IPCT) helps adult patients of any age with chronic health conditions and complex medical needs.  

We work closely with family doctors across North Toronto to offer a range of care and support to those who don’t currently have a primary care provider. Our services include: 

  • Health assessment and treatment 
  • Home safety assessment 
  • Connecting clients to a family doctor or nurse practitioner 
  • Support after a hospital stay 
  • Helping with chronic disease management 
  • Finding free or low-cost services where needed 
  • Helping with digital health tools and virtual care 


Our team includes a range of health professionals who work together to provide personalized support to meet our patients’ specific needs. We offer both in-home and virtual visits.

After your referral is accepted, a member of our team will call you to find out more about your needs. 

We will schedule a health assessment, either in your home or virtually, depending on what works best for you.  

Together, we will decide on a care plan that meets your needs. That plan might include: 

  • Prescribing medication and monitoring your health 
  • Helping you find a family doctor 
  • Setting up appointments with other health professionals like physiotherapists or occupational therapists 
  • Connecting you with other community resources like counselling, education or personal support workers 
  • OHIP card or other health insurance information 
  • A friend or family member for support 
  • A list of questions you want to ask 
  • A list of your medications and supplements 
  • Any other clinic specific items 

You need a referral from a physician or nurse practitioner for a geriatric assessment.

What is an interprofessional primary care team?

Our team is a group of different health professionals who work together to provide a variety of health-care services to people with complicated medical needs.  

How is this different from a family doctor?

Rather than seeing one doctor, you can benefit from the expertise of our whole team of health professionals. You should still have a family doctor who manages your care. But you might also have appointments with a psychologist, physiotherapist and a social worker, for example.  

Is this service only for older adults?

Our team specializes in the unique challenges that older adults with multiple chronic health conditions face. However, we are open to supporting adults of all ages with complex medical needs.
