Accessing Your Health Records
You have the right to access your health records.
If you are a former patient or resident of Baycrest, you can ask for a copy of your health record by:
- Downloading and completing the Consent to Disclose Personal Health Information form (PDF)
- Mailing, faxing or hand delivering it to our Health Records office
To request corrections to your personal health records, please complete the Request for Correction to Personal Health Records form (PDF)
If you are a representative of the patient/resident, requesting release of health records with the patient/resident’s consent or as permitted or required by law, please:
- Provide a signed consent form or proof of legal signing authority
- Bring photo identification and documentation if you are making a request in person or coming to pick up records
A search and retrieval fee of $30 includes up to the first 20 pages of health records. Additional fees may apply, such as the following:
- Copies $0.25 per page after the first 20 pages
- Courier and materials if applicable
There is no charge for health records sent to another physician or care provider.
