The Geriatric Assessment clinic provides comprehensive assessments for older adults with complex medical issues, functional decline or memory concerns.
In our clinic, Baycrest geriatricians offer a holistic approach that treats the whole person, including what we call the five M’s:

You will meet with a geriatrician who will talk to you about your symptoms and medical history.
You will then have a detailed examination to assess both your physical and mental health.
We will work with you to create a treatment plan that will meet your needs. Your plan may include any of the following:
- Referrals to specialty medical services at Baycrest or elsewhere
- Referrals to support services
- Recommendations for your family doctor
- OHIP card or other health insurance information
- All medications in their original bottles or current blister pack (including vitamins/supplements)
- A list of questions you want to ask
- Any medication you might need to take while at the clinic
- Any aids you use, such as a walker, hearing aids or glasses
- A friend or family member for support, if you choose
- Water and a snack if needed
You will need a referral from your family doctor to visit our clinic.
Please fax this referral form to 647-788-2199.
After the assessment, a detailed report will be sent back to your family doctor.
Who should have a geriatric assessment?
There is not one set age for having a geriatric assessment. In general, older adults with more than one chronic health condition, memory problems or frailty can ask their family doctor for a referral to our clinic.
What is the goal of a geriatric assessment?
Our goal is to assess each client to make sure they are receiving the right treatment and management for their specific health conditions. This will help to increase their independence and quality of life.
Can family members or caregivers attend the appointment?
Yes, we encourage family members or caregivers to attend the appointment. They can often provide extra information and a helpful point of view about our clients health and functioning.
