Geriatric Psychiatry Community Service
The Geriatric Psychiatry Community Service (GPCS) is a community outreach program for older adults with mental health conditions.
Our interprofessional team provides mental health services for homebound older adults who are unable to visit Baycrest. Clients may have physical, cognitive or emotional limitations.
We see people with mental health conditions like anxiety, depression and bipolar disease.
Our services include:
- Psychiatric assessment and diagnosis
- Treatment including medication and therapy
- Connecting patients to resources and services
- Case management to help coordinate ongoing support
- Education and support for clients, families and caregivers

After your referral has been accepted, a case worker will phone you to set up first visit.
At that visit, an in-home case worker and psychiatrist will ask you questions as part of an assessment.
We will then work with you and your family to decide on a treatment plan that will meet your needs. Treatment plans may include:
- Medication
- Psychotherapy
- Relaxation techniques
We may also offer case management services that include:
- Advocating for the best services for you
- Ongoing monitoring and support
- Referring and setting up appointments with other services and programs
You can expect to have several follow up visits from our team. Follow-ups can be both in person and virtual.
- OHIP card or other health insurance information
- A friend or family member for support
- A list of questions you want to ask
- A list of your medications and supplements

The GPCS can accept patients who fit the following criteria:
Age 65 or older
Have a psychiatric or suspected psychiatric illness
Unable to attend appointments at Baycrest or requires case management
Live within the catchment area between Dufferin St. and Avenue Rd and between Steeles Ave. and St. Clair Ave.
We can also provide services at Baycrest for people who live outside our catchment area.

Where do appointments with the GPCS happen?
Appointments are usually held in your home or virtually. We can also see patients who live outside our home-visit catchment area at Baycrest.
What kind of help does this service provide?
We provide a wide range of mental health support, including psychiatric treatment, caregiver support and coordinating other services.
Can family be involved?
Yes, we encourage family or caregivers to participate in the appointments and care planning. We also offer education and support for caregivers.
