Older adults in the community who have concerns about their physical health or memory may benefit from a comprehensive assessment by one of our experienced geriatricians - physicians who specialize in a holistic approach to seniors' health.

The Baycrest Geriatric Assessment Clinic sees clients from the community with a wide range of concerns and issues.

In addition to comprehensive geriatric assessments, we provide referrals to Baycrest programs and support services as needed. Our clinic is staffed by certified geriatricians and other healthcare professionals.

Referrals can be initiated by family, friends or health professionals but a medical referral form must be completed and submitted by the family physician who will provide ongoing care to the patient.

A detailed report will be sent to the referring family physician after a discussion of the client's management plan.

  • Where is the Clinic?
    The clinic is located in The Sydney & Florence Cooper Outpatient Clinics area on the ground floor of the Hospital building across from the cafeteria.
  • How do I make an appointment for a geriatric assessment or consult?
